            4. (U) TITLE: Logistics: Quality Assurance Checks of Leased
            Vehicles

            5. (u) OBSERVATION:     Damage to leased vehicles.

            6. (u) DISCUSSION: No "normal" vehicle operator checks were
            conducted on any vehicle. Many vehicles were turned in with
            several quarts low on oil; broken windows, mirrors, dents and
            large scratches and no one was held accountable for the damage.

            7. (u) LESSON LEARNED, If accountability procedures are not in
            place or not enforced, then leased vehicles will be misused.

            8. (u) RECOMMENDED ACTION: Quality assurance checks be required
            daily by the unit motor officer or some one responsible within
            the chain of command. DA Form 2404 should be done daily and
            turned into TMP on a weekly basis.
